Remote Monitoring After Hospital Discharge: A Multi-Site Feasibility Study

Hospital discharge is an important transition in cancer care delivery, yet patients often have limited connection with their care team at this time. This study will explore the feasibility of utilizing a digital remote patient monitoring program at the Hartford Healthcare Cancer Institute with the goal of enhancing a patient's access to timely symptom management leading to benefits in patient engagement, quality of life, and acute care utilization. This project seeks to collect preliminary data to establish the feasibility of implementing a remote monitoring program at the HHC CI. Findings from this study will lead to a randomized, multi-center grant focused on improving care delivery for oncology patients using remote monitoring at hospital discharge: a high-risk period in their treatment continuum.

Criteria:

  • 18 years of age or older
